| A completed program request form is required to be submitted to Health and Wellness Services a minimum of three weeks before the scheduled event. Failure to schedule with the Coordinator of Wellness Services may result in a denial. |

| 2009/2010 Health and Wellness Services Fee Schedule |
| Students are charged for office visits, in-house diagnostic tests, physical exams, immunizations, gynecology exams, medications, supplies, and treatments. Students are not charged for consultations, blood pressure checks, or medication refills. Health and Wellness Services accepts cash, check, VISA or Mastercard or charges can be posted to your student account. We do not process debit cards or flex dollars. |

| Health and Wellness Services only bills BC/BS and Priority Health POS. For other insurances, Health and Wellness Services will provide you with a receipt to submit to the insurance company. Clients needing laboratory services can be drawn at the Health Center and sent to a local laboratory. Laboratory serviced done off site can be billed to the insurance company. Radiology procedures are performed at local hospitals and are billed to the insurance company by a third party billing service. |
