Requesting an Official Transcript

Step 1: Fill out the Transcript Request Form (pdf) and send it to the Registrar office in one of the following ways:

  • Mail: Registrar Office, 1607 Robinson Rd, Grand Rapids, MI 49506
  • Fax: (616) 732-4431
  • E-mail: registrar@aquinas.edu
  • Note: Your signature is mandatory on the form. If it is not an actual signature, the form cannot be processed. Typing your name on the form is not a sufficient signature.
 
Step 2: Pay for the Transcript

Transcript fee: $6 for each copy requested, Emergency fee is $16 and each copy thereafter is $6. Express Mail transcript fee is $24.

  • Pay online
  • Mail check to: Registrar Office,1607 Robinson Rd, Grand Rapids, MI 49506
  • Over-the-phone with Student Accounts at (616) 632-2864 - mention transcript fee.
  • In person at the Registrar office - cash or check is accepted.
 
Requesting an Unofficial Transcript
Unofficial transcripts are given to students that are currently enrolled and are for internal use only.
 
To request an unofficial transcript, e-mail your request to registrar@aquinas.edu with your Aquinas email and provide your AQ student ID number.
