| Requesting an Official Transcript |
Step 1: Fill out the Transcript Request Form (pdf) and send it
to the Registrar office in one of the following ways: |

| Step 2: Pay for the Transcript |
Transcript fee: $6 for each copy requested, Emergency fee is $16
and each copy thereafter is $6. Express Mail transcript fee is $24. |
How to pay: |
- Pay online
- Mail check to: Registrar Office,1607 Robinson Rd, Grand Rapids, MI 49506
- Over-the-phone with Student Accounts at (616) 632-2864 - mention transcript fee.
- In person at the Registrar office - cash or check is accepted.

| Unofficial transcripts are given to students that are currently enrolled
and are for internal use only. |

| To request an unofficial transcript, e-mail your request to registrar@aquinas.edu with your
Aquinas email and provide your AQ student ID number. |
