This form should be completed by students who intend to live in the College residence halls. Aquinas College has a residency policy for all freshman and sophomore students.
Freshmen and sophomores MUST live on campus unless:
They commute from the home of their parents;
They are 21 years old by the opening day of school; or
They are married or have been in the armed services of the United States.
This release form
authorizes Aquinas College's Health & Wellness Services to administer medical
services including immunizations and allergy injections and
to perform routine medical care as deemed necessary by licensed
medical personnel.
Immunity is required prior to registration. Proof of immunization is required. Portions of this form must be completed and signed by your health care provider.
STUDENT TESTING, ADVISING & REGISTRATION (STAR):
This is a mandatory,
day long session of placement testing, advising and registration
of your first semester classes. There is no cost to you; everything
is included. Several STAR dates have been scheduled throughout
the year. General information

to Aquinas life. Our spring semester begins January 11, 2010.
